Telehealth Informed Consent
By submitting an intake form and using Grace Longevity's services, you acknowledge and consent to the following:
Nature of Telehealth Services
Grace Longevity is a health technology platform that facilitates access to independent, licensed physicians who provide telehealth consultations. Telehealth involves the delivery of healthcare services using electronic communications, including asynchronous review of your health history and goals. Clinical care is provided by licensed medical providers affiliated with an independent physician group, not by Grace Longevity.
No Guarantee of Treatment
Completing an intake form or selecting a program does not guarantee that you will be approved for treatment. Prescription medications are only provided when a licensed medical provider determines they are clinically appropriate for you based on your health information.
Provider Review
Your intake information will be reviewed by a licensed medical provider. Depending on your health history, goals, treatment request, and applicable state requirements, the provider may request additional information, laboratory results, or a live video consultation before making a treatment decision.
State Availability
Telehealth services may be available in all 50 states where clinically appropriate and permitted by applicable law. Clinical care is provided by licensed medical providers affiliated with an independent physician group.
Compounded Medications
Medications dispensed through Grace Longevity are compounded medications prepared by licensed compounding pharmacies. Compounded medications are not FDA-approved. The FDA does not evaluate compounded medications for safety, effectiveness, or quality.
Not for Emergencies
Grace Longevity is not appropriate for emergency medical situations. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.

Cancellation & Refund Policy
Grace Care Plan Cancellation
A minimum of 30 days written notice is required to cancel or pause your Grace Care Plan. Notice must be submitted in writing to support@gracelongevity.com. Any charges that fall within the 30-day notice period are non-refundable.
Consultation Fees
Consultation fees are non-refundable. The consultation fee compensates the independent physician for their time in reviewing your health assessment, regardless of outcome.
Medication Fees
Once a prescription has been dispensed and shipped by the pharmacy, refunds are not available for medication. Compounded medications are prepared to order and cannot be resold or returned for safety reasons.
How to Cancel
To cancel or pause your plan, email support@gracelongevity.com with your name, date of birth, and written cancellation request. We will confirm receipt within 2 business days.
